Forms are to be filled out at the beginning of each Scouting year. Applications will be forwarded to the local council office and a copy will be kept by appropriate personnel (i.e. Section Leader, Commissioner, Committee Chair) for response in the event of a medical emergency. Please note that Scouts Canada is committed to respecting the privacy of our members, their families, and our employees, by adhering to the privacy principles set forth in Schedule 1 of The Personal Information Protection and Electronic Documents Act. The purpose of gathering the information on this form is to provide leaders with the information they need to facilitate the activities of youth participating in Scouts Canada program activities and to be able to respond in the event of an emergency. This form is to be completed and signed by the parent/guardian. The parent/guardian should notify the leader if there are any changes to the information on this application during the year.

It is the adult's responsibility to notify/update appropriate personnel of any changes in their medical status or other information contained in this form that may occur throughout the Scouting year. As part of the membership requirement, the volunteer agrees to submit to and provide a police records check clean of any criminal convictions.
